The following information is for patients wishing to make an appointment about allergic problems

There is a questionnaire to be completed and returned to Reception before an appointment is made. This can be returned by personal delivery, post or scanning and sending by email or fax

Dr Cohen has approvals from the Australian Therapeutic Goods Administration (TGA) as an Authorised Prescriber to obtain allergy extracts from USA and Europe for patients having allergy desensitisation treatment

This questionnaire is designed to assist patients in recalling any previous allergic conditions and treatment, and to record their current symptoms and treatment

It is easy to forget such details during a consultation and being able to take your time and make a note of seemingly unimportant details can be most helpful in working out testing requirements, diagnosing your condition and planning for future treatment

It also saves consultation time and allows you to avoid being offered any treatment which has been given previously but was unsuccessful
